Job Description

Job Requirements:

  • Bachelor’s degree in occupational therapy or equivalent from an accredited institution.
  • Additional certifications (e.g., CPR, First Aid, specialized therapy certifications) are preferred.
  • Valid SCFHS license as Specialist
  • 1-3 years of clinical experience in rehabilitation, and/or long-term care settings.
  • Experience working with elderly populations, persons with disabilities, or post-acute care is preferred.
  • Proven experience in developing and implementing individualized care plans.
  • Familiarity with assistive devices and environmental modifications.

Key Job Responsibilities:

  • Perform initial and ongoing assessments of patient’s’ functional status.
  • Design individualized treatment plans based on assessment findings.
  • Deliver hands-on therapy sessions and interventions.
  • Adjust treatment plans based on patient’s progress and changing needs.
  • Educate patients and families about therapeutic activities and adaptive strategies.
  • Recommend and assist with the procurement of assistive devices.
  • Ensure safe and effective use of therapy equipment.
  • Collaborate with team members to develop comprehensive care plans.
  • Document all assessments, interventions, and outcomes accurately.
  • Participate in discharge planning and transition of care.
  • Conduct environmental assessments to identify modifications needed.
  • Contribute to staff training on occupational therapy practices.
  • Promote adherence to infection control and safety protocols.
  • Support patients’ participation in recreational and social activities.
  • Engage in continuous professional development and stay updated with best practices.
