The Diabetes Educator provides comprehensive education and clinical support to patients living with diabetes, empowering them to manage their condition effectively and improve their quality of life. The role involves developing individualized care plans, delivering patient and family education, supporting physicians in treatment adjustments, and ensuring compliance with DOH/DHA and international standards.

Responsibilities

• Assess patient needs and develop personalized diabetes management and education plans.
• Educate patients and families on insulin/oral medications, glucose monitoring, nutrition, and lifestyle modifications.
• Collaborate with physicians and multidisciplinary teams to optimize patient outcomes.
• Ensure accurate documentation, compliance with JCI, DOH/DHA, and infection control standards.
• Contribute to audits, research, and service improvement initiatives.
• Promote patient advocacy, safety, and adherence to organizational values.
  

Qualifications

• Bachelor’s degree in nursing, or any recognized health field.
• Certified Diabetes Educator (as per DOH/DHA guidelines).
• Minimum 2 years’ experience in diabetes education.
• Excellent communication and patient education skills.
