Capital Health is seeking a compassionate and dedicated General Practitioner – Long-Term Care (LTC) to provide comprehensive medical care for patients with chronic illnesses, functional limitations, and complex rehabilitation needs. The role focuses on maintaining patient stability, supporting recovery goals, and working within a multidisciplinary team to ensure holistic, patient-centered care.
💼 Key Responsibilities:
• Provide comprehensive medical assessment, diagnosis, and treatment for long-term care patients
• Manage chronic and acute medical conditions with a focus on comfort, safety, and long-term stability
• Conduct regular ward rounds, follow-ups, and ongoing clinical reviews for assigned patients
• Monitor, review, and adjust medication plans, ensuring safe management of polypharmacy
• Provide emergency medical care for LTC patients when required
• Collaborate closely with physiatrists, rehabilitation specialists, nurses, therapists, and case managers to support coordinated care
• Participate in multidisciplinary team (MDT) meetings and contribute to individualized patient care plans
• Communicate proactively and compassionately with patients and families regarding medical status, care progress, and treatment goals
• Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system
🎓 Qualifications & Requirements
Education & Licensure:
• MBBS or equivalent medical degree from a recognized institution
• Valid and current DOH/HAAD License or Eligibility as a General Practitioner
• BLS/ACLS certification preferred
🩺 Experience:
• Minimum 5–7 years of clinical experience post-qualification.
• Must have experience in long-term care and rehabilitation.
• Experience working within a multidisciplinary care environment is an advantage.
