Job Description
Nurse Coordinator is a competent experienced nurse who consistently demonstrates a commitment to expand his/her knowledge and skill base, works collaboratively with interprofessional healthcare and quality improvement teams in a certain disease group.
The Nurse Coordinator is responsible for coordination of the evaluation and preparation of patients within the relevant disease group (example: pre and post organ transplant, pre and post stem cell transplant, etc…). The Coordinator is responsible for coordinating patient care across multiple areas and levels of care, the outpatient clinic, the inpatient nursing unit and the community in order to provide continuity of patient care across settings.
PRIMARY JOB DUTIES AND RESPONSIBILITIES
PATIENT CARE
- Recognizes abnormal, pertinent findings, and initiates referrals/consults accordingly
- Conduct pre-transplant interviews with prospective transplant patients and families. Interpret eligibility criteria for patients, families, physicians and outside agencies
- Prepare and analyze pre-transplant clinical data and collaborate with other medical personnel in preparation of patients and families for transplantation
- Coordinate pre-transplant preparations
- Ensure patient and family education in all aspects of transplantation
- Compose written transplant summaries and clinic visits
- Corrdinatepost-discharge planning and education for transplant patients
- Compile and review post-discharge data for clinical management of transplant patients
- Provide direct care to transplant patients in the Outpatient Transplant Clinic
- Collaborate with medical faculty, other transplant coordinators, nursing personnel, case management, social workers , transplant pharmacists and transplant dietician and other health disciplines to facilitate meeting patient and family needs. Coordinate and lead the transplant selection meetings and document the decisions made by the team
- Provide on-call service on evenings, weekends and holidays when the transplant Services and Clinics are closed
- Responsible for completing charge capture education and in-service training for their Institute within CCAD. When charges are not captured automatically the nurse will complete within Epic in real time, Perioperative Services within 24 business hours
PROGRAM DEVELOPMENT
- Collaborate in the development of protocols and guidelines for patient management
- Develop, implement and evaluate comprehensive patient education programs
- Participate in the development and execution of quality assurance programs and projects
- Partners with other disciplines to enhance patient care through interprofessional education, consultation, management, technology development, or research activities
- Assure that the Cleveland Clinic Abu Dhabi Organ Transplant Program remains in compliance with the Department of Health Abu Dhabi, Federal Government and international standards
RESEARCH
- Support planning of and participate in clinical research projects
- Collect and analyze data for ongoing clinical research projects
- Incorporate research findings into the Organ Transplant Program practice
PROFESSIONAL DEVELOPMENT / EFFECTIVENESS
- Starts and pursues the journey of becoming a certified clinical transplant coordinator, by the American Board Transplant coordinator
- Successfully completes Transplant Coordinator orientation
- Maintains up to date knowledge of transplant regulatory policies and procedures. Applies knowledge of current practice standards, guidelines, statutes, rules, and regulations into practice.
- Participates in interprofessional team processes through positive actions and open communication
- Maintain knowledge of current practices in transplantation, nursing and healthcare
- Identify professional strengths and areas for growth and demonstrate professional development in forma and informal learning experiences
- Participate in professional organizations in the area of transplantation and transplant nursing and other areas of professional interest
- Participate in local, regional and national conferences on organ transplantation
EDUCATION
- Participate in teaching activities at Cleveland Clinic Abu Dhabi for physicians, nurses, allied health professionals and for students in those disciplines
- Develop and present information to professional groups regarding organ transplantation and organ donation
- Develop and present information to community groups regarding organ transplantation and organ donation
- Participate in the development of marketing information regarding organ transplantation and organ donation
PERFORMANCE MEASURES
- Implements practice protocols competently and consistently
- Establishes patient nursing care requirements relative to specific needs
- Consistent documentation has been recorded by the Transplant Coordinator
- Documentation supports recommendations and actions
- Documents in accordance with Cleveland Clinic Abu Dhabi policies and procedures
- Establishes goals and strategies for meeting discharge and/or continuing care needs of patient, family and/or other care provider based on admission and ongoing assessment. Involves the patient and family in planning.Patients post operative living arrangements are addressed to safely manage health care needs
- Ensures that patient and family demonstrate knowledge of health status, treatments, symptom management, skills, medications and adaptive behaviors gained as a result of teaching interventions
- Safely and competently executes technical skills required for practice.
- Attendance at continuing education classes and/or conferences related to Transplant patient care
QUALIFICATION & EXPERIENCE REQUIREMENTS
Education
- Minimum of a Bachelor’s degree in Nursing
- A Master’s degree in Nursing is preferred
- Post graduate /certification in relevant nursing specialty is preferred
Experience
- 5 years of clinical nursing experience
- Progressive nursing experience with demonstrated success in the relevant nursing specialty (example: caring for transplant patients in ICU or ACU)
- Clinical nursing experience ACU and ICU is preferred
- Two (2) years of relevant nursing specialty experience (example: transplant) is preferred
Certification and Licensure
- Valid and current national license is required
- BLS is required
Job Specific Skills and Abilities
- Strong oral and written communication skills
- Proficiency with Microsoft Office suite
- Strong interpersonal skills
- Ability to effectively manage multiple responsibilities simultaneously
- Fluency in written and spoken English
- Experience with electronic medical record is strongly preferred
- Ability to speak and write in Arabic is preferred
