The CMO – Discharge Coordinator, reporting to the Case Manager, supports safe and efficient patient transitions by leveraging a licensed nursing or medical background and experience in case management or discharge planning. The role focuses on coordinating patient flow, ensuring continuity of care, and communicating effectively with multidisciplinary teams, patients, families, insurers, and external facilities. The CMO – Discharge Coordinator must also adhere to M42 and comply with DOH policies, procedures, guidelines, and standards.

Responsibilities

  • Coordinate and manage patient discharges to ensure safe, timely, and efficient transitions of care.
  • Support case management activities by assessing patient needs and planning appropriate discharge pathways.
  • Monitor patient flow and collaborate with clinical teams to maintain continuity of care across the hospital.
  • Communicate effectively with multidisciplinary teams to align discharge plans and resolve barriers.
  • Liaise professionally with patients, families, insurance providers, and external healthcare facilities to facilitate smooth transitions.
  • Maintain accurate documentation in EMR systems and ensure compliance with regulatory and quality standards.
  • Prioritize and manage multiple discharge cases simultaneously in a fast‑paced environment.
  • Provide culturally sensitive support, with Arabic language communication preferred for patient engagement.

Qualifications

  • Licensed Nurse with a valid professional nursing license or relevant medical background.
  • 2-3 years of experience in discharge planning, case management, care coordination, or a similar healthcare role.
  • Proficiency in EMR systems, Microsoft Office, and strong understanding of healthcare regulations and documentation standards.
  • Strong communication, negotiation, and interpersonal skills, with Arabic language proficiency preferred.
