We are seeking a skilled and dedicated Inpatient Pharmacist to support safe, effective, and evidence-based medication use across inpatient services. The role includes prescription review, clinical collaboration, patient counseling, medication safety, sterile IV compounding, and participation in the Antimicrobial Stewardship Program, in compliance with DOH, JCI, and hospital standards.
Responsibilities
- Review, verify, and accurately dispense inpatient medications in compliance with DOH, JCI, and hospital standards.
- Provide patient counseling on medication use, dosing, precautions, side effects, monitoring, and safe storage.
- Ensure timely medication dispensing to support safe and continuous inpatient care.
- Identify and resolve medication-related problems such as interactions, duplications, contraindications, dosing errors, adverse drug reactions, and renal or hepatic dose adjustment needs.
- Collaborate with physicians, nurses, and multidisciplinary teams to optimize medication therapy and patient care.
- Participate in clinical pharmacy activities including medication reconciliation, profile review, therapeutic monitoring, and evidence-based recommendations.
- Contribute to the Antimicrobial Stewardship Program by supporting appropriate antimicrobial selection, dosing, route, duration, de-escalation, and IV-to-oral conversion in line with local guidelines.
- Prepare, label, and verify sterile compounded preparations, including intravenous admixtures, using approved aseptic technique and regulatory standards.
- Perform accurate pharmaceutical calculations for IV medications and other sterile preparations.
- Support medication safety practices related to Look-Alike Sound-Alike (LASA) drugs, high-alert medications, and safe compounding and administration.
- Maintain proper medication storage, monitor expiry dates, and document pharmacy interventions and clinical activities accurately.
- Participate in ongoing training, competency assessment, and continuous professional development in inpatient pharmacotherapy, antimicrobial stewardship, and sterile compounding.
Qualifications
- Bachelor’s degree in Pharmacy with valid DOH license.
- Minimum 5 years of experience in an inpatient or hospital pharmacy setting.
